NAME "Hail to the Chief" Small Scale 2008 Electronic Packet - How it Works
It's easy to use. Just click on the name of the page in the Table of Contents that you would like to view and allow time for it to appear on your screen. Some of them are quite large and may take several seconds, so please be patient! You will not be able to fill out and submit these forms on-line, so you will have to print the ones you need and mail them to the proper address. If they are forms to be sent to the NAME Office, you also have the option to fax them (317) 571-8105 if you are using a VISA or MasterCard. Please pay attention to the dates and mailing addresses on each form.

First be sure that you have Adobe Acrobat Reader version 5.0 or better installed on your computer. If you don't have reader installed now, visit adobe.com and download the current version. If you already have Adobe Reader on your computer, it is best to upgrade within Reader. (Open Reader, go to HELP, and click on CHECK FOR UPDATES.)
AOL USERS -
We have found there is sometimes an issue with Adobe Reader and the AOL browser. We suggest that you do NOT use the AOL browser, but minimize it once you get online and open a different browser such as INTERNET EXPLORER or FIREFOX, OPERA, SAFARI (To download any of these free browsers, click on its name)
It also helps to open Adobe Reader first, then your browser.
If you still can't open the file successfully, try saving the file to your computer and opening it offline. (To save use right click with PCs, CTRL-click with MAC)
